Applicants must provide copies of:
PROOF OF IDENTITY FOR ALL HOUSEHOLD MEMBERS
(Texas Driver’s License, Texas ID, US Passport, Military ID, Permanent Resident Card or Matrícula Consular)

PROOF OF CITIZENSHIP OR LEGAL RESIDENCY FOR ALL HOUSEHOLD MEMBERS
(US Passport, US Birth Certificate, Permanent Resident Card, Certificate of Naturalization or Certificate of US Citizenship)

PROOF OF INCOME FOR THE LAST 30 DAYS FOR ALL HOUSEHOLD MEMBERS 18 YEARS OR OLDER, INCLUDING:
Wages, Social Security income, unemployment, SNAP/TANF, retirement, disability income, child support, etc.

CURRENT ELECTRICITY, GAS AND WATER BILL Utility assistance for electricity, gas and/or water Service or repair of your HVAC system
